- Fargo-Moorhead:
The
Family HealthCare Center (a CHC) serves a community composed of
over 17 cultures of resettled refugees, in addition to the regular
population. Strategies needed to be developed to accommodate for
the phase out of a family practice residency program by UND. Community
discussions were
conducted in Fargo-Moorhead on May 11-12, 1999. A Briefing
Packet compiling secondary
data was produced for the community
site visit and a follow up report developed in June 1999. Strategic
planning with the FHC Executive Director and the North Dakota
staff of the Community Health Care Association was conducted in
December 1999 to facilitate next steps.
- Bismarck:
I
attended a community meeting on October 28, 2000 in Bismark to
assist CHCA in facilitating a group working on a HUD Continuum
of Care Plan, one component of which is striving toward a Health
Care for the Homeless project.
- Outcomes:
- The
community meetings secured public support for FHC.
- Fargo-Moorhead:
Follow
up meeting with the community to discuss future stategies for
FHC held in October 1999 during Community Health Center Week.
- Bismarck:
The
subcommittee working on health care for the homeless adopted 100%
and Zero Disparities as part of their goal on October 28,
1999. Objectives, strategies and time lines/next steps were set
at meeting through my facilitation.
